I am requesting a report that shows diagnosis for a selected period.Colums needed are Patient name,Encounter no and diagnosis code (ICD9 or ICD 10)

You might take a look at OpenEMR’s Reports menu. Between them, all the reports that come standard have most all of the basic data such as you require. And just about every report’s output format is a table that you can copy/ paste into a spreadsheet and delete the columns you don’t want.
That of course is an informal, slap-dash workaround in case you do not want to build the report yourself using a MySQL query, or pay somebody to code it for you.
