We thought to add in the medical problems a radio field that enables the doctor define when diagnosis is made by him or by others and then he’ll record the name of the other clinics/ doctor.

Sound like a useful thing to add. Would consider placing it at bottom of all input for all the issue types.
A radio field will likely get way too long. And would instead consider a select list for this that, which lists the active list of providers(see elsewhere in codebase how this is done) and also allow an empty choice there. And when create a new issue, would default it to the physician that is logged in (and if the person logged in is not a physician, then default to empty).
This would especially be useful in the setting where somebody else besides the physician is entering in the diagnoses or other issues so can still associated the original physician with it.
