Personally, I think this is a really poor practice that should never be permitted in a patient chart. I think even for electronic medical records that it is better to amend the record to state that the entry was made in error instead of "deleting" an error. The fact the delete buttons are so close to the save buttons almost always contribute to more unintentional deletions that intended deletions.
