Are only Administrators allowed to add CPT4 codes or does the accounting and front office acl qualify for the procedures?

Not completely but pretty sure, admins are the only ones who can add codes to the system. I have only ever done it as an Admin.
And, unlike ICD-10 and other codes, CPT codes are a special case because one does not usually install the full and complete codeset, only the ones used by the practice. In case you haven’t seen these wiki pages they might come in helpful:
