CMS 1500 Box 24I/24J

Hello,

I am attempting to figure out how to get Box 24J (and qualifier, 24I) to populate for a CMS 1500 2/12 form.

These are submitted to insurance but are being kicked back due to the need to specify a G2 qualifier in 24I and also the provider ID (for Gateway insurance) in box 24J.

I looked at the HCFA Options but do not see anything additional for the 24I/24J boxes.

I need to input 2 provider numbers specific for the insurance company that will print in those boxes at time of generation.

What guidance can you offer? I read through the following post, but not certain it applies to what I’m trying to do:

I feel like a dork.

I believe I found what I was needing.

Administration > Practice > Insurance Numbers

I’m filling those values in and will test.

EDIT: That was what I needed, and it works as expected.

Administration/Practice/Insurance Numbers should hold the NPI for the rendering physician, which goes into the unshaded portion of Box 24J.

I think the rejections resulted from the shaded part of Box 24I & 24J being empty. See attachment.

Shaded 24I or ID Qual. should contain G2. Shaded 24J is usually the non-NPI or legacy number for the rendering physician. See this.

Uncomment the snippets for other insurers, so ID Qual. & legacy number will not populate.

It would be a lot easier to send e-claims, see this.

If the practice is not using a clearinghouse, such as Office Ally; be aware of total segment counts when sending 837P’s.
