yes I’m using System Operation windows 7

I am sure that the problem I am facing this in the database, whenever I import my database into the system openEMR the problem prevails.

Hi @vaz,

That’s not a standard OpenEMR SOAP form. The SOAP form usually has a blank text field for Objective. Your form looks like it is using a list from list_options maybe. What does your code look like there?
