Can anyone help me to get and generate CCM report for patient.
I am checking various link but did’t get any reference to create and generate CCm reports.

I will be great if anyone help me to get the proper solution.

If you are referring to CCD or Transition of Care documents generated from Care Coordination Module with help from our CCDA generator then, there’re several forum posts around you can refer to for help.
