This MU is closely related to CPOE, Clinical Summaries, and Health Information Exchange. Currently, what is missing is a referral summary, which can be created by converting the paper referral form to an electronic format. Then it can be ordered and added to Clinical Summaries as a separate section.

Not sure I understand. Referrals are already stored electronically, in the transactions table.

I would move it to CPOE since it’s a required item there. See the MU requirements for CPOE: http://www.openmedsoftware.org/wiki/Computer_Physician_Order_Entry.
This would be easier for generating Clinical Summaries and doing HIE as well. We want a referral summary that can be included in Clinical Summaries.

No, there is no quick example. Analysis and design of appropriate conversion is part of the deal with any enhancement, and in many cases will be non-trivial.
