Regarding “The reminders are sent to provider’s Messages box, which are then route to the patients” - I expect the provider (doctor) will not want to be in that loop. But if they are, see my comment in the previous thread which observes that Patient Notes already serve as an inter-office messaging system.
The VOIP part sounds very cool.
