can you use? openemr for ems

It certainly can; it’s only software so you can use it for any purpose it suits. And it’s FOSS so you are free to use it for any purpose you can modify it to suit.
If your intended use requires particular certifications you should check that OpenEMR can meet the requirements. However, at my company we have supported its use in many different clinical specialties including in an EMS setting. The codebase was heavily modified, since many of the data forms and workflow requirements were quite different from those needed to operate a general medical practice which OpenEMR is designed to do.
In my experience as a UI and Customer Support pro (not a developer) your biggest obstacle will be in formulating a clear and specific plan for what you want your EMS application to do. Then figure out what OpenEMR can already do so you do not duplicate features. Then code the remaining capabilities.
