Bug: Can't untag a document / encounter pair

There is a minor bug someone could fix. In Document Manager you can Tag a document to an encounter, but if you pick the wrong encounter you can NOT Un-Tag it.

Hi Brady ,

We found the same issue in openemr development code base ( 5.0.1 dev) and we did fix for this .
Here is the fix

Please check this and review it . Let us know if any changes requires .
