Most of my U.S. clients generate claims with OpenEMR. CPT4, HCPCS and ICD9 codes are supported. Referring physicians can be put into the Address Book and selected in patient demographics.
Conversion, when done, is usually a challenge.

after checking several open source EMR, i have decided to stay with openEMR under linux. this has the most active support with no strings attached.
the billing module works including the referring provider.
it needs some customization to meet your requirement but works out of box for small practice.
