Is it possible to apply sensitivity to forms or specific fields in forms? I see it’s there for encounters, but this would be a useful way to control access to specific sensitive information that the patient might enter through the portal or provide during the encounter.

That’s a good idea but much more fine- grained (i.e., would take lots more computing to enforce) than the approach taken in OpenEMR.
The ‘sensitivity’ setting you describe applies to the sensitivity of the data that any staff is permitted to see. Which is why an encounter for the purpose of discussing those issues can have its sensitivity elevated above the default, and staff’s sensitivity rating can be set at a level to allow or prevent access to those encounters.
Restricting staff access to pt info is usually accomplished by restricting the user’s clinical role ACL (Access Control) permissions or the Patient Privacy module.
