So you went to administration → Forms-> Layouts and selected demographics, added a new field, selected listbox, and pointed it to the new list you made under the list tool?
Do you need just a single start and single end date? Or the date at which they change levels, etc?
If you select add field under Who/contact, etc. and select type text-date it will add a date picker field.
