1500/X12 line 24d (Explain Unusual Circum...)

Hi
   We have run into a situation which requires that both X12 and 1500 (line 24d) have the Explain Unusual Circumstance filled in.  On the 1500 this information goes just above the CPT/HCPCS code.

Any Idea as to how to get this done?

You will have to modify the code to get this done.

HCFA format : BOX 24 D for cms 1500
1. In fee sheet form (…/interface/forms/), provide text boxes for cpt or hcpcs or both to take that input, just like NDCs.
2. Create a respective field to store this text info in database.
3. in gen_hcfa_1500.inc.php Map it before NDC.

The shaded line will allow for the entry of 61 characters from the beginning of 24A to the end of 24G. Box 24 d shaded portion takes Explain Unusual Circumstance  notes, Anaesthesia start stop, ndc qualifier, code, quantity and units. VP – HIBCC codes, OZ – GTIN, CTR contract rate, or tooth numbers and areas of the oral cavity
NOte: Do not enter a space, hyphen, or other separator between the qualifier and the number/code/ information.
