X12 and New Encounter fixes applied

I have just checked in fixes for the following:

1. freeb: Patch from Alfonso to send correct receiver ID in X12 transmissions.

2. freeb: Fixes for invalid "emergency" flag in X12 transmissions.

3. freeb: Added hospital admission date for inpatient claims in X12 transmissions.

4. openemr: Added a second date to the New Patient encounter form, for onset-of-illness or hospital admission.

And added a new field onset_date to the table form_encounter. To update an existing implementation, you should add the colum to the end of the table in the Database Administration module.
