I have successfully established connectivity with OpenEMR. I am able to post the form on wordpress which goes on CMS Portal requests in OpenEMR.
On posting the form, wordpress shows no response message or error but I can see the request coming in OpenEMR.
Can you please tell me what am I missing?

There is a exact same notice for list_backup_id.
Can you please tell me why this might be coming?

Not sure I understand the original question. In any case there are many harmless PHP Notice messages logged by OpenEMR, and the one above seems unrelated to the CMS Portal.

After I post the data using forms in the wordpress portal, I don’t get any response. In OpenEMR, I can see the data coming in the requests section. I want to know, how can I display the submitted data to user.

The Ninja Forms plugin will prescribe any response, which will be minimal. Probably what you want to do is go to some suitable page. Currently there is no feature to show portal users what they have submitted after the fact.
