I would think that the majority of physiciains use oral thermometers. Should’t oral be the default value?

This is an elseif decision structure but ALL of the temperature forms default to the last else statement which is "axillary". I.E. all of the statements evaluate to false and the last default value is selected.

Also all of the fields that are left blank are entered as zeros.

Most of these (or all) values should be rounded to 1 significant digit. Such as:

30.1

not

30.07222478

All of the extra digits do not have statistical meaning, are mostly useless and decrease readability.  One exception would be pediatric weights on children less than 2 years of age.  Here 2 significant digits have meaning.

I do wish we had spots for supine, sitting and standing blood pressures, and the additional info text box.  These could come after the save button to allow more rapid tab-keyboard entry.
