Thank you. These tables do not look like the tables for the LBF forms. Were these constructed differently? I like the ROS check form and would like to make it part of a comprehensive exam form. Is there somewhere that I can see the layout of these forms?

The answer is: Start digging into the FORUM for HELP, USERS and Developers. Search with you key words, it will give you more info on your topic than you could ever imagine, but the answer is there.

It is not that easy to do radio button items and have a symmetrical layout. That is why I built the NT form with NationNotes and not with radio buttons. You may end up very frustrated and losing a lot of valuable time. Instead of inserting the radio button ROS form into the NT Comprehensive Exam form, I will make another suggestion.
If you want other Groups such as GI and symptoms such as Dysphagia, do so in the Layout Editor for the NT form and add the symptoms as Components. Clicking a radio button and a Component both require one key stroke.
Next year, after you are comfortable with OpenEMR, you can play with the radio buttons and modify the NT form to your heart’s content. For now, it is urgent that you get started soon. You can’t get started if the H & P forms are still being built.
I am in awe of your sang froid. If I were in your shoes, I would be a basket case now. If MU Attestation is no longer a goal, then the serenity is understandable.
The perfect should not be the enemy of the good (enough).

On the Open-EMR WIKI-page search for FORMS got 8 hits for more info on FORMS.
From there you can make another search with the info gathered. Changing ROS-form is usually done with the old form and renaming the form in a name more suitable for you.
The LBF-Visit forms are more user friendly and I would suggest to follow the discussion of the Ophtalmic-forms. This is a more recent discussion about what to do and where to find and place the results.
Hope this will be of some help. Otherwise FSGL can give more instructions. The lay-out of included FORMS in OpenEMR can only be viewed in a local install if some changes of the lay-out is the goal.
In Demo you can view them, use them and install them through (just installed the PAIN Graphic form in Demo V4.1.2):
Administration => Other => Forms => Register => Install DB => Give the Name Administration or Clinical.
Many more FORMS (with instructions on how to change) are available, but you only can find them and copy them to the right Directory and use them in a local-install, or Web Installation. This feature is not available in any DEMO of OpenEMR.

If you have some time, start experimenting. LBF-Visit forms is one way to go. Forms is the other way.
JUST DON’t GIVE UP! Once you find what you are looking for it is there in Open-EMR to be made available.
There are instruction on both type of FORMS somewhere in Open/EMR or in the Software itself. Just take some time to experiment and let us know of your results. Always nice to help someone who wants to improve OpenEMR for their way of handling the medical work-flow.

It is not the Tables that are different (Yes they are different, but the USER won’t see the difference). They just work in the back-ground and “gather” and keep information on the right spot and connected to the right Client.
The Lay-out of the form and the information you want to get into the Form for your client might be something to be studied before it works.
At the end the USER gets an almost smillar view with different options to fill the FORM used.
