Visit Form shows old data?

Hello,
I have created my own layout based visit form. When i fill the form for a specific patient for an encounter that happened, then when i go to that encounter or make a new encounter to see or to fill more data, the fields that i had filled before are empty! Is there a way so that the previously filled data could be shown?

No, the data i enter are shown in the information of the encounter but not when i open the form again.

LBF is encounter specific. If you make new encounter and open the ‘same’ form, you are creating a new record with no data. If you want prior encounter information to be copied to new encounter, you will need to use plugin facility and copy the data.

If you are not seeing LBF data for the original encounter, there is something wrong with your setup.

An option would be to open an old encounter/edit the old LBF form an continue with another date/the new date.

Or use general text and store it in a nation note predefined text.

Example: General text for a weight-loss clinic might be:
Client visited dietician
Client can keep the diet
Client started with more exercise
Client need to has a lot of constipation, need to make it more laxative.

Screenshots are of a SOAP note, but it works the same for a LBV form. The LBV form should be on the top of the screen of the particular encounter. In Past Encounters and Documents, a mouseover will also shown the form.

Would suggest that you duplicate your LBV form in the 4.1.2 (3) Demo so that we can take a look.
