maybe try stepping thru each release to see how far you get?
i.e. from 3.0.1 to 3.1.0 to 3.2.0 etc

It may be possible to leapfrog from 3.0.0 to 4.1.2 if the old version is listed in the drop down menu at localhost/openemr/sql_upgrade.php.
If upgrading the database, serial or leapfrog, does not work; XAMPP may need upgrading as well.
The version of XAMPP has changed considerably; 1.7 then, 1.8.2 now. Try installing the latest package, import the old database, then upgrade.

Leapfrog upgrade(3.0.0 to 4.1.2) should work from the sql_upgrade.php script:
Also will need to migrate the directories, so would use these instructions to do the upgrade:

If the sql_upgrade.php script is not working, then will be helpful if you paste the output of the script here.
