Upgraded from 2.8.3 to 3.0.1

So far only one reported case; having people globally go through two different upgrades increases potential of mistakes. I plan to run through the upgrade from 2.9.0->3.1.0 to see if I can recreate this bug. If I can, then will rec separate upgrades. If I can’t then will await other reports (taking an evidence based approach here). The important thing is that thanks to penguin8 we know there is a potential problem and we know the solution; so we can provide better support if somebody else reports having this problem.

   Not sure if this helps any, but so far the 3 systems I’ve had this issue on were all originally installed as version 2.8.x, maybe it’s some kind of side effect of earlier upgrades from 2.8.x to 2.9.x ?  I think having GACL emebedded in OpenEMR will eliminate the problem from here on out, it’s just getting over that version 3 hump & everything should be smooth from there on.
