Tele-Health in the US and CMS

There are a host of fascinating changes being considered in Washington:

  • One flat Office visit fee schedule for level 2-5 (just hit level 2 documentation levels - no more E&M coding level nonsense).
  • Ability to add a code for medically complex decision making
  • Ability to bill for communication/chart review leading up to visit (they are even considering “Voice-only” interactions).
  • Ability to bill for communication which results in NO visit being needed - ie preventative care.
  • Stripping down global periods
  • Payment for telemedicine based on e-visit duration.

From OpenEMR’s vantage point, having a built-in telemedicine platform and network could be a huge bonus. Is anyone work on this? If any of the vendors out there have deployed something similar, would you be willing to share your work? I’m sure there are many providers in the community who would benefit from having e-consults available and consultants willing to participate.

CMS is looking for comments up til Sept 12, 2018 on these rule changes.
