Current version of Gentoo.
Current cvs version of openemr and freeb.
Zirmed clearing house.

Problem : freeb is not including the medicare insurance information correctly and the secondary insurace is not there at all when I generate HCFA files.

What other correct information do you need to be included in the medicare insurance information? And the secondary insurance information is not usually included when printing HCFA forms, you have to modify /formatbin/hcfa.pl or /openemr/library/freeb files for it to be included.

The changes will be extensive and difficult to debug.  Based on my experience you do not have to have secondary insurance included if you were to bill medicare alone, but if you have to send to secondary insurance, for example, medicaid or supplemental carrier. then you need to change certain fields in the hcfa.pl to have it included.  I did some changes to hcfa.pl when I submit medicare crossover for patient who has both medi-cal(calif. medicaid) and did not meet deductible for medicare. It works for my purpose, but this is not a permanent solution.  If you are in California and need to send for Medicare Crossover claims to Medi-Cal only, I can send you my modified hcfa.pl.

I have a script named zirmed.php which generates claims in a format that Zirmed accepts.  I then copy the screen contents to a text file and upload.  It has worked very well for me so far.  I intended to improve it to upload directly from openemr via ftp, but I have not done this yet.  I don’t think zirmed.php in cvs is up to date.  I will update it if needed and help you set it up if you are interested.  It does not use freeb at all.
