I recommend using the "Fee Sheet" encounter form as the superbill. You can modify interface/forms/fee_sheet/codes.php to customize it.
It is practice-wide, but I suppose you could modify it to include different versions of codes.php depending on the logged-in user.

I want to enter patient info, and then print out a customized superbill that I hand to the doctor with the patient data on the top, then the doc checks off what he did to the patient, then we enter that into the computer.

Im thinking I will have to create a form that allows me to go in and choose all the cpt codes and diags I would want to appear on a printed enounter form. Then at the top I would grab the patient data and print it out.
