Standard Measures Report issue

Running v4.1.0. When Standard Measures report is run, the following are missing: total patients, denominator, excluded patients From Measurement: Weight, Education: Weight, Education: Nutrition, Exercise, Measurement: BMI.
Is this a bug or what am I missing to input in the front end to get results? I have entered weight for patients, as an example. TIA.

For those results, the information needs to be collected in the following places:
**Measurement, Weight = Encounter > Vitals
** Education, Weight
is for children and adolescents (2-18), and it will appear after information is completed in Encounter > Vitals
Education, Exercise is for children and adolescents (2-18), and it will appear after information is completed in Encounter > Vitals
Education, Nutrition is for children and adolescents (2-18), and it will appear after information is completed in Encounter > Vitals
Measurement, BMI is for children and adolescents (2-18), and it will appear after information is completed in Encounter > Vitals

You can see which rules are triggered by certain actions by going to the Administration menu, then selecting Rules. A simple two to see how it works are the Tobacco rules. If you look at Tobacco Use Assessment, it’s target is the Lifestyle - Tobacco field in History. It must contain a value to be cleared from the patient.

From there, select Tobacco Cessation Intervention. This rule will look at the Lifestyle - Tobacco field for a value of “current” to trigger an intervention for that patient.
