When checking any options under "Review of Systems", the entire list is added to the encounter summary. This includes all yes, no and N/A choices. Is it possible to have only the selected options summarized under their respective categories?

I’m glad you brought it up.  The form making tool does not address the possibility of N/A.  The forms that it creates list checkbox groups as “Positive for … Negative for …”  I need to fix this.  You can use the form making tool (formscript.pl found in the contrib directory) to easily make your own ros form.  I am going to add some new features soon so I will fix the lack of N/A also.

Most (probably all) of the N/A choices are based on sex.  It would be nice for the form to check the sex in demographics and only show the appropriate questions.

Also, for the legally compulsive USA, it is important to record answers to questions as yes, no, or not asked.  Questions that were not asked should either not display at all (my preferred option) or display as "Not asked".  This applies to null fields if present.
