Review of Systems Checks Form

When I try and use the Review of Systems Checks form and click save I get the following error:

ERROR: insert failed: insert into form_reviewofs set pid = 1,groupname=‘mygroup’,user=‘myuser’,authorized=1,activity=1, date = NOW(), poor_appetite = ‘on’, additional_notes = ‘additional note’ (Table ‘openemr.form_reviewofs’ doesn’t exist)
