I have been testing openemr in a “sandbox” machine and I deleted out the list “Layout-Based Visit Forms”. I made a new list with the same name, but of course, it does not interact with the Layout module. Is there a way to reinstall this module or link the new list “Layout-Based Visit Forms” to the Layout module?

Our understanding is, initially you have deleted the “Layout-Based Visit Forms” list and again tried to recreate it. The reason it does not get mapped may be because the New list option enables to create a list but not “Layout-Based Visit Forms” list. Recreating “Layout-Based Visit Forms” list through phpMyAdmin or mysql command line with old values will resolve your issue.
