Prior_auth_form table missing - Hcfa fails

I thought this would be useful, since I spent a considerable amount of time finding out how to solve this problem. If you are unable to create hcfa files  because a table named prior_auth_form is missing, you need to go to the "Administration" page in openEmr, then "Form Settings", then register the "Prior Authorization Form" and enable it.
