I’m looking at the database in the CVS focusing in on the patient. Being a nurse the patient is the center of my attention. Anyway, one immediate simple area of improvement that would add value towards better patient reporting and satisfying CCHIT Ambulatory criteria “AM 01.04” is to add a status field to table patient_data with enumerated choices “active” and “inactive”.
The value from CCHIT and quality assurance perspectives is that it indicates which patient charts can be ignored (exempt from reporting). In NJ we have lots of patients moving to Florida or the afterlife. Failure to inactivate their record results in my institution getting dinged for not ordering mammograms or HBA1C levels on them, i.e. lack of quality care.
