I have a pediatrician who has developed about 30 age appropriate forms for ~15 well child and ~15 sick child visits. I would like to convert his paper forms to HTML.
It would be nice to dynamically pick the appropriate peds form based on the sex and age of the child from the demographics.
Has anyone made progress opn the growrth charts? I remember seeing some earlier threads on this?

I did one like that, under contrib/forms there is a form called well_child_care it atomaticaly takes the child age and shows only the appropriate checks for the visit.
