New Encounter form example-template

I’ve just committed to CVS a new form called ‘example’ in the interface/forms folder. It’s a sample form which is a little easier to understand then taking apart another existing form. My hope is that people will be more apt to create their own custom forms if they have more clear directions and code to work from.

The example includes pop up calendars, use of jQuery, and CSS. It also now includes a Print button so the form can easily be printed.

I have other ideas on how to make the forms better and will try to make some time. But for now the example-form is my best attempt to improve this area of OpenEMR.
