PQRS codes cannot be entered without the penny. If you use zero for the fee, nothing will show up.
If the Standard Measures Report don’t have the numerators/denominators, you must get them manually from Reports/Clients/Clinical using the diagnoses and the PQRS codes. Some of the Clinical Quality Measures are not hard-coded, i.e., Diabetic Eye Exam, 2022F. That is why the Standard Measures Report cannot be used for 2022F.
This link will tell you how to configure the Clinical Reports to give totals.

Before we go off on tangents, is this exercise in preparation for MU Stage 1 Attestation for a practice in Internal Medicine or Family Practice?
If that is the case, only the 3 Core Clinical Quality Measure are required. Therefore, concentrate on the measures for Hypertension, Smoking Screening and Adult Weight Screening; see this .pdf.
Your link above pertains to the PQRS Pilot Program, which is completely different from the EHR Incentive Program (MU Stage 1).
Sherwin Gaddis noted in the other thread that Adult Weight Screening has been hard-coded. The Adult Weight numerator and denominator should appear in the Standard Measures Report.
The Smoking numbers are in the AMC Report.
The numbers for Hypertension should be in the Standard Measure Report, provided the diagnosis of Hypertension (with the ICD-9 code) is in Issues/Medical Problems and BP’s have been recorded for all Hypertensive patients in 2013.
If all the numbers are available, there was no need to mess about with the work-around. There is no need to fool around either with the 3 Alternate Clinical Quality Measures nor with the 3 Additional Clinical Quality Measures!
Are the all the numbers in the Standard Report and AMC Report?
If the work has been done for you already and you feel generous after getting the $15K from CMS, consider donating dineros to the project because there is quite of bit of work to be done for MU Stage 2.

1. Check that the rules are in place, see attachment 1.
2. Check that the Passive Alerts are in place, see attachment 2.
3. a. Hypertension requires the diagnosis and ICD-9 codes in Issues/Medical Problem.
b. Weight requires that plus height in Vitals. BMI is calculated automatically for you.
c. Smoking status is entered in Issues/Lifestyle. Regarding Smoking Cessation Intervention, I am not positive that has been hard-coded. On the Attestation for 2012, if you did Smoking Status, you got credit for Smoking Cessation Intervention. To be on the safe side, prepare an Alternative Clinical Quality Measure just in case you need it. See attachment 3.

Try unchecking all of them, save & re-checking Hypertension, Weight, both Smoking and save again.

We’re closer I got some to show up. I can only check off weight and tobacco (2 ea). Hypertension makes the report turn blank. I must choose one provider only.

Very strange behavior indeed.
The reports should be run for only one practitioner because attestation is per one practitioner.
Perhaps with an overnight rest and reboot in the morning, maybe the strange behavior will go away after you play with Alerts again tomorrow. If your local machine remains recalcitrant, we may need to give a shoutout to Brady or Kevin.
Another thought: run a report with Weight & Smoking, then run a separate report with just Hypertension. There is no law against having 2 pages of print-out to support your attestation.
Still another thought: noticed that you used Internet Explorer. Try Firefox instead. IE can be wacky at times.
Question for you: where are the numbers for Smoking Cessation Intervention generated? I experimented with the Counseling part of Lifestyle in Issues to no avail. The correct number of smokers pops up as the denominator, what about the numerator? What did you do last year?
If you don’t get the correct numerator for Smoking Cessation, there is a PQRS code for Diabetic foot care and you will then have to use the work-around for the NQF 0056 Clinical Quality Measure.
