Another suggestion is try 1 and 6 in first object while all other objects on the same row as 0 and 0.

Could someone please compile what the conclusion is for the Layout for LBV forms? Four Column’s was stipulated some time ago. If placed as 0 and 0 followed by 1 6, you get the last column filled and jump to the next row…. is this conclusion correct?

To change the Tab > Miscellaneous do the following:
Go to Administration > Other > Forms > Change Category from Miscellaneous or if it is blank, into Clinical same Capital and lower case characters or if the category is a blank name it Clinical.
