Does OpenEMR alllow for Multiple seperate acounts for multiple locations? 2 or more fanchises, for example? Or should a copy of OpenEMR be used for each location?

Multiple sites are supported. They are each completely distinct from each other requiring separate databases. I support 3 different clients from one server.
You can access the configuration to add additional sites at:
https://yourdomain.com/openemr/admin.php
When it comes to patching and backups and such there will be some additional steps but searching through the Wiki will provide the details.
