Does OpenEMR support multi-tenancy? There seems to be evidence of this in the OpenEMR 4.0 branch with the sites directory but looks like there are 2 databases required.

Dunno what multi-tenancy means, but current code has “multi-site” support. Each site does still have its own database. See this thread, especially post #15 on page 2:
