Can you explain the topography, where is the Server located and where are teh two facilities located and what it is that you actually want to achieve.
Without know ing any of the facts, here is the simplest solution that I can think of for your situation.
Install an instance of OpenEMR 4.0 in the clouds.
Once installed, log into it as Administrator and go to the Administration Menu. From here select Facilities and then enter the information on the two facilities.
Then go to Globals ( also from the Administration Menu) and select the tab marked Features. About half way down the list you can tick mark the box against the line where it says Restrict Users To Facilities.
Having done this, you can then add your users and be sure to assign them the correct facility that they belong to. Wehn they log on, they will be logged on the the Facility where they belong.
In this way, you can set up several Facilities without any additional changes being required and as Adminsitrator you can get consolidated Financial and other reports.

Glad that the explanation was helpful.
If you have a Static IP Address at teh location where the Server is located, then from across the street, you would simply open the browser and type in the URL as
http://xxx.xxxx.xxx.xxx/openemr
In the example given above xxx.xxxx.xxx.xxx is the Static IP address and I am assuming that the folder from where OpenEMR is launched is called openemr. If that is not correct, please replace openemr with the correct folder name.
If you do not have a STATIC IP Address, then i would recommend using a free service such as DYNDNS.ORG
You will need to open an account with DYNDNS and select one the free Domain Names that they have. Next you will have to install a DYNDNS client on the Server. What this client does, is that everytime your IP Changes it notifies DYNDNS. They in turn propogate the change to all the DNS Servers. In this way, you will always have access to the Server regardless of how many times a day the ISP changes your IP address.
The last step that you need to do, is to do port forwarding on the router in the facility where the Server is located. You MUST use a STATIC IP Address on the SERVER for Port Forwarding to work.
