To edit the current Review of System forms would require mods to files in interface/forms/ros/ (or interface/forms/reviewofs/) and in the sql table form_ros (or form_reviewofs).
Note new forms can be created via several mechanisms, which are explained here:

It never hurts to ask 
Generally, to get coding/customization done in OpenEMR, there are several options:
1. Do it yourself (the community is usually happy to provide guidance)
2. Ask volunteer(s) to do it
3. Hire a professional
Guessing you won’t get any bites with volunteers on this, but you never know, so is always good to ask or bring up this kind of thing in the forums. If considering a professional, we maintain a list of professionals on the OpenEMR website, which I am happy to provide the link for.
