If you are a specialist sending reports to others, you are better off using commercially printed letterheads. When printing any report, just adjust printer’s margins or set your printer with correct margins. Some printers will even pull letterhead from a different tray for 1st page and regular printer paper for subsequent pages.
To create soft copies, use pdf printers like bullzip which do the same effects.
These would require no code changes.

A default letterhead will appear when printing Report from Patient Summary; albeit to the left margin, not centered as done traditionally.
An alternative is Templates for Patient Documents, which will be available in 4.1.3. A logo can be added easily.
