I registered my OpenEMR for Medicare & Medicaid EHR Incentive Program in 2013.
For 2015 validation process I need to verify EHR certification number. Is there is any changes or I need to confirm same certification #
2.2.d is the correct reason to choose for the Hardship Exception.
The instructions made no mention of supporting documents, but Tony has made this available in case CMS asks for it subsequently. There does not appear to be any contact information for asking questions. You can try the same email address to ask about supporting documents.
In December & January the Payment Adjustment & Hardship Information webpage stated that the deadline was March 15th. Apparently it has been extended to July 1st. That’s good for procrastinators.
So to confirm, if we have been attesting for meaningful use for 2012, 2013 and 2014 using openemr, then we cannot attest for 2015 using openemr 4.2? This is for ophthalmology .

Because you are scheduled for Stage II for 2015; it’s required that OpenEMR be certified for the 2014 Edition, which is not the case. This requirement is for all specialties.
Because users are unable to attest for 2015 due to the lack of 2014 CEHRT, we must apply for the Hardship Exception. The exception will take you off the list of practices that will be fined a reduction in the 2017 Medicare reimbursement. Your attestation failure (through no fault of your own) is “excused” by virtue of the Hardship Exception.
You cannot attest for 2015 because the reporting period is one full year for Stage II, which means that you must have AMC reports from Jan. 1, 2015 to Dec. 31, 2015. For Stage I attesters, they need a reporting period of any 90 days in 2015. The attestation deadline is March 11, 2016. Even if OpenEMR achieves 2014 CEHRT, through divine intervention ahead of that deadline; users, regardless of stage, still cannot attest.
