It seems that the database has grown & sql queries are starting to slow down performance on v6, I would like to configure MariaDB for Big Query Settings. Is it supported?

I’m not familiar with those options but I’ve looked in on them, and I’m pretty sure you won’t suffer data /loss/, so I don’t have a reason to recommend against experimenting.
However… I’m not wholly familiar with how OpenEMR handles under heavy load, but it doesn’t strike me as likely that the problem is going to be solved by treating production activity like large reporting queries. If this solves the problem, that’s good to know; if it doesn’t, you may need to consider either reviewing the slow activities for DB improvement (how are we for indexing?) or you may need to consider hardware upgrades (more RAM, more cores, faster drives).

From experience 160MB database is not large right?
No, that’s a pretty small database. Where are you seeing performance problems?
It’s not performance problems after all, it seems that there might be missing data
