A blank frame usually means a PHP error occurred. It’s very important to look in your Apache error log, wherever that may be, to find out what the error is. That is ALWAYS your FIRST step. Without that you’re just groping in the dark.

Question remains… I can see the left menu with old left_nav V4.1.7 and working, but left_nav.php of V4.2 is not.

Suggest that you install 4.2.0 (guide ), since you have no data as of yet; rather than troubleshooting the upgrade.
Seriously consider the security problems as they relate to HIPAA with hosting. The less you have under your own control, the less security you have.
