We have created a number of LBF custom forms. when we generate a report, only one of those forms will show up on the generated form. I can see no difference in how we created the LBF. How can I make those other LBFs show up on the generated report that is printable?
Sounds like a bug may be lingering somewhere. Need to know several things:
1) OpenEMR version and package (ie. source vs. xampp vs. ubuntu vs. appliance)
2) Your operating system
3) Which specific report?
If it’s the Patient/Client->Medical Record->Report:
When you select what to include in the above report, can these forms be selected in the ‘Encounters & Forms:’ section? Or are you selecting, them and are they are still not showing up in the report after click ‘Generate Report’?
1) OpenEMR version 3.2.0 xampp
2) Windows XP
3) It is this Patient/Client->Medical Report->Report, yes they can be slected in the ‘Encounters & Forms’ section. Either way I try it, both by selecting the entire encounter, or selecting the forms individually, the only thing that will show up on the generated report is the Date in parenthesis.
3 out of 8 of the LBF we have created will show up appropriately on the generated report, but the other 5 just show the (date).
We did install the latest patch last night, no luck.

If you go to Administration -> Lists -> Layout Based Visit Forms, what is in the ID column for each of these? I’m thinking there may be a bug where something in there is confusing the software.
Otherwise, you may need a pro to troubleshoot your site.
You know what I actually wound up doing, the only thing I could figure out was to completely erase the corrupt forms, from the LBF list in “lists”, enter it as a new LBF again, and the went into “Layouts” and completely remade to forms. Now they so show up on the printable generated report.
