Layout Based Forms ERROR

Suddenly cannot create new Forms, when I create the forms a blank area shows up under the CLINICAL list in the edit forms section and no name etc, cannot add new LBF forms

I’m using OpenEMR version 5.01

I’m using: Chrome

I’m using: Linux on AWS

Could you please share the error logs to analyse further.

where do I find the error logs?

I did a new install and when I try to create a new LBF same issue, it says LBF form does not exist but I can see it in the database.

Have you tried the below steps.

  1. Administration->Layout->New Layout.
  2. Enter the form details and mention Access Control where you want to see your form.
  3. In Edit Layout section, specify data type and fields for your form.
  4. In Encounter Summary page, the new form created is shown under the specified section.
