hey,
I use command line with a text editor (vim or jed) for all development. For me this is advantageous since can then developer on any computer that has an ssh terminal (always on linux, and can install putty on windows). Some others use graphical gui IDE’s, so depends on what your comfortable with. We have a OpenEMR Developer Appliance here:

I don’t use a IDE. Instead I use several text editors - KWrite and Gedit on Linux. TextMate on Mac.
