Hi @Jessica
It’s hard to tell where you went wrong but I suspect you have missed a few steps.
Here’s an unfinished doc I’m close to completing, but I think the page I link below might have what you’re looking for.
[https://www.open-emr.org/wiki/index.php/OpenEMR\_Questionnaire\_-\_Import\_Questionnaire](https://www.open-emr.org/wiki/index.php/OpenEMR_Questionnaire_-_Import_Questionnaire)

Also, it may be Too Much Information and hard to get through, but the Dev who invented templates and the questionnaire form has posted a lot about them in the forum.
See these posts:

Here is the main forum post about the questionnaire form:

> [@Questionnaire/Assessment feature implementation instructions](https://community.open-emr.org/t/questionnaire-assessment-feature-implementation-instructions/18870):

> Warning see [Questionnaire/Assessment feature implementation instructions - #30 by sjpadgett](https://community.open-emr.org/t/questionnaire-assessment-feature-implementation-instructions/18870/30) New encounter questionnaire instructions: [Questionnaire/Assessment feature implementation instructions - #5 by sjpadgett](https://community.open-emr.org/t/questionnaire-assessment-feature-implementation-instructions/18870/5) Though many will be creative in different ways to develop a questionnaire, I’ll show a couple ways to get started using NIH tools. Note the following websites: [LHC-Forms Questionnaire App](https://lhcforms.nlm.nih.gov/lforms-fhir-app/?server=https%3A%2F%2Flforms-fhir.nlm.nih.gov%2FbaseR4) Is an available application populated with many test patients and questionnaires. The other main post on Questionnaires:

> [@Implement FHIR Questionnaires in Portal via Portal templating](https://community.open-emr.org/t/implement-fhir-questionnaires-in-portal-via-portal-templating/18833):
>
> As a start to full implementation of FHIR API Questionnaire and QuestionnaireResponse resources I’ve decided to work out the Questionnaire rendering and save as both the FHIR resource for API and answers for internal analytics perhaps useful for CDR engine. @mdsupport put me onto an excellent project to make this much easier to implement namely the NIH projects starting with the rendering engine at [LHC-Forms: A Form-Rendering Widget](http://lhncbc.github.io/lforms/index.html) The basic integration for this is creating new tables like qu…

Good luck!
