Hello, I am having a problem with the eye exam form. If I use it, I can’t sign the query.
Is there any special procedure with this form?
How can I make it firm after using it?

Please describe the experience, and include screenshots of where it stopped functioning?
As it happens I attempted on my demo to e-sig the eye exam and was also unable to get it complete the e-sign.
Of interest that eye exam did not appear on the report (main menu: Reports/ Visits/ Encounters) which should list the signed and unsigned encounters and forms.
I’m working on OpenEMR v5.0.2 (2) on AWS cloud, I believe.

I do not know why this problem is, but I solve it by replacing the form with this one that a friend gave me. Now you can sign, but the eyes form continues to absorb other forms.

Yes, but isn’t perfect !!! this form works poorly with other forms.
now I’m trying to get mail notifications to patients working. You are using v5.0.2 (5) on win 10 with xampp and mercury. Mercury works fine but openemr doesn’t load the output messages into any table. I even have the mail cron running and nothing !!!
