How to record who referred pt to us?

How do i save info as to which doctor or hospital referred the patient to me?
Can we create this feature when a pt arrives to the clinic and on the page where we create a new patient.?

Thank you. This is a much needed feature to see from where the clinic/hospital is getting patients from.

I think this will also serve the purpose and the text also says “Referral Source” and one can add another referral source on the spot by clicking add.
