but i need blank forms demographics details should be empty, i will need to print the blank demographics details available for patient personal details,how is possible?

Hi Sourav…
Workflow -

  1. Create New Patient
  2. Complete the demographics…I think you are from India, We dont need Insurance much in India, so leave it blank
  3. Create encounter - write the chief complaint in the “Consultation Brief Description Box”
  4. Save and then in the encounter summary page fill out forms as required. Do the vitals, Take history, do examinations and fill them out.
  5. Create a fee sheet - (please configure the fee sheet before as I have added my custom CPT code like 0001 for Consultation and then in the list options have added that in the fee sheet). This helps in selecting the consultation automatically and populate the fee sheet with the consultation charges.
  6. In the fee sheet itself you will give the diagnosis by ICD10 or ICD9 selection box
  7. Then in the fee sheet, you will find CPT4 highlited, click on that, it will show you the diagnosis you made right now and tick the J (justify) and P (Problem) checkboxes. This will include the ICD into the fee sheet also associate the encounter with the issue created.
  8. Now, go to patient summary and on the right hand pane, you will find prescription. Add the prescription drugs and print it our for the patient.
  9. While writing the prescription drug, you will find an option at the bottom saying " Add to medication list" you can select yes to that, then the medications you are adding in the prescriptions will be added to the medication list in the issues part of the software and you can associate them with the current encounter too.
  10. Next, in the patient summary screen, you will notice report in the top.click on that.
  11. there you can select the demogr4aphics, the encounter you did, the diagnosis and everything else you need. Just click the checkboxes.
  12. Click “Generate Report”
  13. A new window with all the forms and the details will appear. Click “printable version” above and then another window appears with printable format. right click and select print. you are done.
  14. Next bill the patient with Payment/Checkout/Billing in the fees menu.
  15. The Encounter over. Do another appointment
  16. Give all the reports to the patient in a file. and the happy patient goes home.

If you need to print blank demographics page, go to reports in the left menu, click " Blank Forms" and then select Demographics. DONE
Encounter forms cant be printed blank as of now.
